The cat being larger in diameter can offset its lower density to some extent and improve the flow capacity in the meanwhile. I heard a few companies do that in some other applications.
Most sport cat bodies are circa 5" (120-125mm) Dia. BCS here in the UK do 5.5"/140mm dia sports cat. They have tested flow of this with their decat pipe and delta is neglible. I have one fitted to my EWG m135i with stock midpipe and MPE. Its been working fine for 2 years although it's in combo with jb4.

Long term issues? Yeah.... I burn through more gas because I love the sound of free revving and holding gears much longer than I should. My car's ego is too big because of all the compliments on how great it sounds.
Seriously though, I haven't had a single issue. No creaks, rattles, smells or anything negative to report after 3+ years and 25k miles. I know it's an expensive piece of metal, but it installs easily, makes real tune-able power, and makes the N55 sound like pure f-ing sex.
