I think what you are missing from the Apex guide is this sentence "5mm front spacers or larger may be required with certain aftermarket suspensions, see below."

38-12.5= ET 25.5 which is still off of ET23 (28-5) they recommend for aftermarket suspensions with 255 tires. Add the 5mm of extra tire of a 265 and your friend would need a minimum of 7.5mm extra worth of spacer for a 20mm total spacer.

Not sure why but many coilovers have the perch right where the tire goes (instead of above as OEM) which limits wheel/tire width inwards.

Another thing to try are narrower tires. OEM 265s are actually 245mm at the tread.

BTW I have 265s up front on OEM susp. and on 18*9 M513 wheels.

Camber wont help with strut clearance. The geometry of the strut to the wheel does not change with camber. Camber will only help with outer fender clearance.
You are right. Reading it again today I missed the 5mm "or greater" part. I had him move to an 18mm spacer clearance is close but it's not hitting anymore I think (3-5mm range), considering moving to a 20 and calling it a day.
